What is Rhinoplasty?

Rhinoplasty is a surgery performed to correct the appearance of the nose that makes the person unhappy. In this procedure, bone and cartilage curvatures (deviations) that prevent the patient from breathing can also be corrected.
 
What kind of process awaits patients who want to have rhinoplasty?
It should be underlined that patients who want to have rhinoplasty must first undergo a face-to-face examination. It is important to evaluate the internal and external structure of the nose and skin type. Which Procedures Can Be Combined with Rhinoplasty?
Non-invasive applications such as fillers and fillers to the facial area are performed very frequently, provided that they are planned before surgery. Also, permanent lip lift, prominent ear correction, and eyelid procedures can be performed in the same session as rhinoplasty.

Open Technique Nose Surgery

It is the most preferred method among nose aesthetic techniques. In open technique rhinoplasty, an incision is made in the skin inside the nose and between the nostrils to access the nasal cartilages and bones. The biggest advantage of open technique rhinoplasty is greater control of the nasal dorsum and tip of the nose during surgery. After the nose shaping process is completed, the skin is adapted to its place and all incisions are repaired.
 
Closed Technique Nose Surgery
In closed technique rhinoplasty, there is no incision at the tip of the nose, that is, in the columella, and all incisions are made inside the nose. In closed technique rhinoplasty, access to the tip of the nose and the back of the nose is more difficult than in the open technique, and therefore the possibility of need for re-operation (revision) is more frequent. Noses that do not require intervention at the tip of the nose and have a slight arch can be shown as suitable noses for closed technique rhinoplasty.

Revision Rhinoplasty Surgery

After rhinoplasty surgery, asymmetry in the nostrils, irregularity on the back of the nose, curvature of the nose, swelling in the arch and arch after rhinoplasty, drooping at the tip of the nose, not having the desired thinness of the tip of the nose, difficulty in breathing after rhinoplasty, etc. Corrective surgery performed for various reasons is called revision rhinoplasty. Depending on the current situation, revision nose surgery can range from a simple bone rasping to complete nose reconstruction. If there is not enough tissue during the second nose surgery and it is needed, cartilage may need to be removed from behind the ear or from the rib.
 
When Should Revision Surgery Be Performed?
The long-term results of the first nose surgery can be fully seen within 1 year. For this reason, one should generally wait 1 year for a second nose surgery. There may be some exceptions and noses that require minor intervention can be treated in 3-6 months.
 
Preparation Before Aesthetic Nose Surgery
  • Some medications may interact with anesthesia or their side effects may occur with surgery. Especially aspirin and derivative drugs should be discontinued one week before rhinoplasty surgery.
  • There is no harm in women having this surgery while they are menstruating.
  • Smoking negatively affects wound healing. For this reason, it is recommended that it be stopped as early as possible before surgery and not used after surgery.
  • There should be no flu infection such as cold or sore throat before the surgery. If it happens, the surgery is postponed.
  • If the surgery is before noon, nothing should be eaten or drunk after 24:00 at night. If it is in the afternoon, you can have a light breakfast, taking into account the 6-hour fasting period.

How is Nose Aesthetic Surgery (Rhinoplasty) Performed?

During the surgical procedure called nose aesthetic surgery or rhinoplasty, different surgical techniques are preferred depending on the content of the aesthetic problem in the nose, the tissue characteristics of the nose, the patient's expectations from the surgery, and whether there are any disorders such as sinusitis, intranasal curvatures or intranasal flesh growths that are planned to be intervened in the same surgery. can be done.
 
Aesthetic nose surgeries are performed under general anesthesia, except for special cases, and the duration of the surgery lasts between 1.5-2.5 hours, depending on the content of the intervention and the technique to be used. The surgery duration may be longer in patients who have previously had nose surgery and in cases where the sinuses need to be intervened during the surgery or a piece of cartilage needs to be removed from the ear or rib.
 
The basic principle of nose aesthetic surgeries is to separate the nose skin and the mucosa lining the inside of the nose from the bone and cartilage structures with the help of instruments applied entirely through the nose or a small part of it through incisions made in the skin compartment between the two nostrils, and to repair the bone and cartilage structures of the nose using different techniques and technologies. After the desired shape is given, the skin and mucosa are placed back in place. In aesthetic nose surgeries, no direct intervention is made to the skin covering the nose, except in special cases.

Healing Process

The healing process begins with the removal of tampons. After two days of rest, patients are able to do their daily work. During this process, mild to moderate bruising may be seen around the eyes. However, with the technique we apply, we do not see any bruises extending to the eye area. The nose becomes edematous after the cast. Although edema resolves quickly in patients with thin skin type, this process may take longer in patients with thick skin.
 
After 1 month after the surgery, 75-80% of the edema disappears. The targeted shape and full recovery are achieved one year after the surgery. During this process, it is normal to experience edema in the nose area, which occurs in the morning and disappears in the following hours.

Things to Consider After Aesthetic Nose Surgery

Applying ice every hour for 30 minutes for two days after surgery will be beneficial.
Bleeding in the form of leakage is normal for the first three days after surgery.
There is no harm in taking a warm shower (2-3 minutes) without wetting the nose area after the surgery.
Starting from the second day after the surgery, you can eat normally, except for salty foods and alcohol that require very hard chewing.
You can brush your teeth with a soft brush.
Regardless of whether there is a tampon or not, the inside of the nose should be washed with plenty of sea water and immediately afterwards, cream should be applied to the nostrils to soften the clots (every hour while awake).
Blowing your nose should be avoided for the first three weeks and the nose should be cleaned with sea water.
After the surgery, you should lie in the head-up position until the bandages are completely removed.
Be careful not to sleep face down or on your side if possible (travel pillows can be used).
You can use supportive pillows produced for patients with nose surgery while sleeping at night.
The plastic cast on the nose is removed at the end of the 1st week (7th day).
After the first week, taping can be applied for a week (7 days).
After the second week, perform the massages described for 3-4 minutes every hour for three months.
For the first 3 weeks, we recommend that you spray the recommended hypertonic saline solution into each nostril, two sprays, 6 times a day, and then apply a thin layer of the recommended softening cream into the nostrils from the inside.
There may be bruising/swelling under your eyes that may last for a week or so. It is recommended that you avoid sun exposure with bruises.
It is recommended to do a light massage with 'Arnica gel' twice a day for 1 month after the surgery. It will help you relieve edema and recover faster.
If you had surgery during the summer, the surgery area should be protected with factor 50 sunscreen that is renewed every hour as long as you are exposed to the sun. You can wear a hat.
Glasses should not be worn for the first month unless otherwise stated. You can use contact lenses (including sunglasses).
Avoid swimming in the sea or the pool for the first 3 weeks.
Feeling like flu and postnasal drip are normal for 3 weeks after surgery.
We recommend that you avoid sexual activity for 3 weeks after surgery.
After surgery, the swelling on the nose subsides at the end of the 3rd month and returns to its normal position in approximately 6 months. In thick skin, this period may extend up to 1 year.
Swellings do not disappear from everywhere at the same time, this can create a feeling of asymmetry in your nose, your nose may look crooked one morning while it is straight, do not worry.
Numbness at the tip of the nose for up to 3 months and stiffness for up to 6 months is normal.
Foods such as parsley, pineapple and green tea help relieve edema.
After the surgery, if possible, we have check-ups at the 1st week, 2nd week, 1st month, 3rd month, 6th month, 1 year and once a year. (If you are out of town or out of the country, you can send photos or videos)
